F4N Board Member Survey

Hello all! I have set this up so that when people respond, I'll get an email letting me know who responded (based on the initials selected) so I can hunt down anyone who hasn't completed the survey, but to preserve privacy & encourage honesty, the end result will go into two reports... one without the initials & one with initials. The intent is to look at the one without initials only, but in the event that someone mentions something that requires one-on-one follow up, we will get someone from outside to get the initials from the one with them (I am thinking Dave's admin (Toni) or something along those lines.)
As this survey is instead of our December one-on-ones, please take the time to give full and complete answers, even if they are long or verbose. This is our opportunity to improve & do better & small or creative things could very well be the things that revolutionize the experience for everyone.
